Medication Therapy Management Program Pilot

Abstract

MEDiC is an organization composed of interdisciplinary student-run free clinics at the University of Wisconsin School of Medicine and Public Health. At one of our seven clinic locations, we have recently implemented a Medication Therapy Management (MTM) program. By introducing a pharmacist as a member of our care team, patients now have the opportunity to receive a wide range of clinical services that focus on medications and medication-related problems. Thus far, we have received positive feedback from the patients seen in MTM clinic, and we have begun screening patients who are candidates for MTM services. We are continually developing new ideas and making improvements to the program in order to best serve our patients while fostering MEDiC’s mission of interdisciplinary teamwork and teaching.

Scott Pearson, University of Wisconsin-Madison School of Pharmacy
Scott Pearson is a pharmacy student at the University of Wisconsin-Madison School of Pharmacy in the class of 2016. He graduated with honors from the University of Minnesota in 2012 with a B.A. in Biology, Society and Environment. His undergraduate honors thesis research investigated the use of omega-3 and omega-6 polyunsaturated fatty acids in the treatment of cardiac arrhythmias. He is passionate about interdisciplinary patient care and plans to pursue a pharmacy residency focusing in ambulatory care. In his spare time, he coaches middle school basketball, plays soccer, and is an avid fan of all Minnesota sports teams.
Joanne Kuznicki, University of Wisconsin-Madison School of Pharmacy

Joanne Kuznicki is a pharmacy student at the University of Wisconsin-Madison School of Pharmacy in the class of 2018. She graduated with honors from the University of Illinois at Chicago (UIC) with a B.S. in Biological Sciences. During her time at UIC, she was involved in research with the Department of Psychology examining anger on electromyographic activity and implicit measuring of distinct emotional states. She also had the opportunity to spend a semester in Copenhagen completing a medical biotechnology and drug development program, exploring new advancements being made in global pharmaceutical research. After graduation, she hopes to work as a pharmacist in the community setting and precept future students.
